[MUSIC]Welcome to Equipped, using the best toolsfor communication, thefourth module in health literacy andcommunication for health professionals.In this video, we will be discussing clearhealth communication and what it means.I'd like to start out this module with abrief reminder.A person's diagnosis of having limitedhealth literacy is to be treatedas protected health information, or PHI,and should be entered into thepatient's health record only so that theorganization's personnel can do theirpart to use appropriate materials forhealth messages tailored to this person.Good health communication starts byremembering that it needs to be a loop.By that, I mean that one way communicationis not always effective becauseyou don't know the intended message wasever received and understood by thereceiver.That takes some extra effort.The next steps of good communicationinclude practicing aUniversal Precautions approach while wecan't tell by looking.Creating a shame-free environment.Using medical interpreter services in theperson's preferred language.Asking for a full set of the person'sconcerns at the outset of the visit.Then it continues with eliciting theperson's prior understanding of theirhealth issues by asking, what do youalready know about your condition?And the use of verbal and nonverbal activelistening techniques.Let's look at two patient perspectives.The first is to determine what a person'spreferred communication style is.>> One thing to keep in mind when youenterinto a communication situation with a deafperson, is that, alot of, what a lot of people tend to do,is they either freeze, and they don't knowwhat to do.Or they try to dig back into their pastexperiences and say, well this onething worked with this one deaf person, sothat's what I'm going to attempt to do.So that leads to situations where either adeaf person is given aninterpretor when they don't need one orwhen they don't want to use one.Or a person comes really close and willstart speaking very loudlybecause that's what the hard of hearingguy they met once before used.But, in many situations that just comesacross as being very hostile andaggressive.And not only that, you're also speakingloudly to the point ofviolating HIPAA, because people around youcan hear this person's private healthinformation.So, when you're put in that situation whatyouwant to do is realize that yes, this maybe a challenge, but you, first thing youfigureout is, what that person's preferredcommunicational style is.So rather than digging back and thinkingokay, howdo I interact with this person, just askthem.They'll be happy to tell you, becausethat'llmake the interaction a lot easier for bothparties.>> The next patient perspective refers toaccess to communication toolsin compliance with the AmericanDisabilities Act, also known as ADA.[BLANK_AUDIO]In this video a pharmacist isdemonstrating acomplex set of steps for proper tohelp your sonbreath better for when he has an acuteasthma attack.How to use?>> How to use?That is a very good question.Because taking this medicine can save yourson's life.This medication is given through aninhaler.Has anyone in your family used an inhalerbefore?>> No.>> Oh, well, with the prescription, youwill get an instruction sheet,but I'll go ahead and explain it to youright now as well.So, you're going to have the inhaler andthe first thing that you're going to needtodo is remove the plastic cap and make surethat there's no dust on it.So next, if it's the first time you'reusing it or youhaven't used it in two weeks, you'regoing to have to prime it.Now, to prime it, you're going to shake itfourtimes and then you're going to spray itfour times.Always away from your eyes.And then to use the inhaler, you're goingto shake it very well.You're going to breathe out deeply throughyour mouth.And then you're going to hold the canisterwith the mouthpieceat your mouth, then close your lipstightly around it.Then I want you to breathe in slowly anddeeplythrough the mouthpiece and then press downon the container.Try to hold your breath for tenseconds, remove the inhaler, and breatheout slowly.If you were told to use two puffs, waitone minute and then repeat steps threethrough seven.And then at the end, put the cap back on.Do you have any additional questions?>> Once for asthma.>> Mr. Brown, that should be once.Only one time.If you see once are you thinking once, 11?>> Yes.>> That is why it is a very good idea thatwe review the prescription with you.Eleven times is much, way too muchmedicationfor your son and it could seriously harmhim.Be very sure he only inhales the medicineone time.>> Si, gracias.>> Advanced directives can be verycomplicated and confusing in anysituation.The following video shares how importantthey are.[BLANK_AUDIO]>> Congratulations.>> [LAUGH].>> Your first pregnancy is very exciting.>> Yeah, wow, we are really pregnant.>> huh, for real pregnant.>> [LAUGH].>> Yes you are.There's a lot for you to think about rightnow, but you should remember.Even in a healthy pregnancy, in a youngerwoman, there aremany, many things that can go wrong due toyour advanced age.>> She's 36.>> Exactly, and with your heart condition,you are at higherrisk of complications, so there are somethings you should consider.>> What kind of things?>> Is everything okay?>> Yes, for now things are fine.However, both of these conditions arepotentially life threatening.I recommend an advanced directive and adurable power of attorney.A C-section is a major surgerywith many associated risks and possiblecomplications.Even in a healthy pregnancy, without yourrisk factors.A colleague of mine lost a woman just lastyear.Excessive bleeding, and nothing they coulddo to stop it.It's unusual, but it happens.So it really is important for you to haveyour ducks in a row.I recommend an advance directive or DPA,maybe a DNR too.Here's some brochures with some examplesfor you to look over.Make sure to schedule your nextappointment on your way out.Again, congratulations.[SOUND] Congratulations.Your first pregnancy is always veryexciting.And I realize, sometimes a bitfrightening.>> Yeah it is.>> And it involves a lot of planning.>> But we have nine months right?Right, ideally, [LAUGH] some of the thingsthat we're going to talk abouttoday might seem a little scary, but theyare all part of the planning.And are only ever used in a very fewcircumstances.Because you are over 35 and because youhave a heart condition, you maywant to consider an advance directive aswell as a durable power of attorney.>> During the exam, you said everythingwas fine.>> And it is.I recommend these steps to all mypatients.Pregnancy has a lot of unknowns and it isbetter to discussthese situations when everything is goingwell and when everyone can think clearly.>> What are we supposed to be thinkingabout again?>> An advance directive is a document thatoutlines what you wantdoctors and medical staff to do if yourlife is in danger.You can decide whether you would wanteverything and anything medicallypossible to be done to keep you alive, orif you would only want certain things.Writing it down makes it easier for us toknow what you want us to do in anemergency.>> Well, I dont know what I would want.>> I don't think she should be thinkingabout that stuff, that can't be good forher.>> It is a lot to think about.But better to do it now, rather thanhave decisions made for you if it becomesnecessary.A durable power of attorney is somethingelse you should consider.>> We're not exactly married.>> A durable power of attorney allows theperson of your choice to make healthcaredecisions for you in the event that youare unable to make them for yourself.A C-section may seem pretty routine, butit still is a major surgery.>> Hm.Do you need to hire a lawyer?>> Not necessarily.I can give you some information to takehome with you and read over, along withsome examples of an advanced directive, adurablepower of attorney, and a do notresuscitate order.You can take your time to look them over,and when you're comingin for your next visit, I can answer anyquestions you might have.How does that sound?>> Scary, but helpful.>> Everything is fine, and hopefully itwill stay that way.But just in case, it is important to beinformed.Do you have any more
